Removing under-eye bags and dark circles with lower eyelid blepharoplasty by Dr. Nichlos
When we meet people, they look into our eyes first. Pronounced under-eye bags and dark circles can easily give the impression that we are tired or exhausted, perhaps even that we stayed up at night or hardly slept, although that is not the case. Sometimes they also wrongly make the face look sad or older than we actually are.
Understandably, many people decide to have their lower eyelids lifted and the bothersome under-eye bags removed. This small procedure is among the most frequently requested operations at our specialist practice in Frankfurt and is well suited to visibly rejuvenating your appearance, opening up your look and giving it back its youthful freshness. What is a lower eyelid lift?
The lower eyelid lift (medically called lower eyelid blepharoplasty) is used to remove excess skin and fat in the area of the lower eyelid that are responsible for dark circles or under-eye bags, which are perceived as bothersome for aesthetic reasons. The goal is to make the face look younger, fresher and more vital again.
The main reason for the loosening of the tissue is the loss of elasticity and firmness of the skin through the aging process. External factors such as stress, an unhealthy lifestyle, lack of sleep or a genetic predisposition can also contribute to the development of dark circles and under-eye bags.
In our guide, you can learn more about the causes of under-eye bags and what really helps against them.

Experiences with the lower eyelid lift at Dr. Nichlos
What are the benefits of a lower eyelid lift in Frankfurt?

Specialist for lower eyelid lifts in Frankfurt
Dr. med. Emmanouil Nichlos has more than 17 years of experience in lower eyelid lifts and aesthetic eyelid surgery.
With a fine sense for the sensitive eye region and modern treatment techniques, he achieves natural, harmonious results. Dr. Nichlos personally looks after freshly operated patients. In the first 24 hours after the operation, he is available to them for questions or problems.
Who is a lower eyelid lift suitable for?
The lower eyelid lift is suitable for patients who are bothered by under-eye bags, dark circles or sagging skin under the eyes and want a fresher look. Whether the procedure is the optimal solution for you is best assessed by Dr. Nichlos in a personal conversation. In our experience, good results can be achieved above all with pronounced under-eye bags, excess skin on the lower eyelid and sagging skin under the eyes.

Which lower eyelid lift methods are there?
In modern plastic and aesthetic medicine, there are two methods for lifting the lower eyelid:
Incision at the lash line (transcutaneous)
With this method, the specialist makes a very fine incision along the lash line. Through it, he can remove excess skin and protruding fatty tissue as needed, tighten the muscle if necessary and even shift fat downward. The latter makes it possible to smooth the transition to the cheek in the case of a pronounced tear trough. The scars usually heal inconspicuously.
Approach through the conjunctiva (transconjunctival)
In transconjunctival blepharoplasty, Dr. Nichlos places the incision not on the outside but on the inside of the lower eyelid. This has the advantage that, as a rule, no external scars form and the natural eyelid tension is preserved. The method is well suited to younger patients with under-eye bags who do not have significant excess skin. Which method is ideal for you depends on your individual starting situation and your wishes. Feel free to schedule a non-binding consultation with Dr. Nichlos.
Can the lower eyelid lift be combined with other procedures?
Yes, in many cases this even makes sense. Looking at the lower eyelids in isolation during treatment would be wrong. For a harmonious result, it is important to also include areas such as the forehead, brows, cheeks and upper eyelids, as they influence one another. Useful additions to the lower eyelid lift are the following procedures, and a facelift can be part of it as well:
Upper eyelid lift
The most common combination at our practice in Frankfurt. Dr. Nichlos lifts upper and lower eyelids in one go, ensuring a well-balanced result overall.

Brow lift
Drooping eyebrows further reinforce the impression of tired eyes. With a brow lift, Dr. Nichlos opens up the look by gently raising the brows.

Fat transfer
Sometimes the problem is not too much tissue but too little. In the same procedure, Dr. Nichlos can place your own fat in the tear trough or cheek area to create a natural transition. So-called nanofat is particularly effective here. This specially processed form of autologous fat is rich in stem cells and growth factors.
Unlike regular autologous fat, nanofat does not primarily aim at volume but at regeneration. It thickens the already extremely thin skin under the eyes, stimulates natural collagen formation and is thus able to visibly reduce dark discoloration. This effect can be achieved neither with filler nor with a lift alone.
Botox and hyaluronic acid
A surgical procedure is not always needed to optimize the result of a lower eyelid lift even further. Minimally invasive methods such as Botox and/or hyaluronic acid also lend themselves to this. Dr. Nichlos uses them, among other things, to fill in missing volume under the eyes or at the temples, to make the transition from lower eyelid to cheek even more harmonious, or to smooth crow’s feet and fine expression lines around the eyes.
Dr. Nichlos is happy to discuss with you personally which treatment or combination makes the most sense for you.
What happens before the lower eyelid lift in Frankfurt?
If your under-eye bags and/or dark circles bother you and you would like a solution through a lower eyelid lift, we invite you to a non-binding consultation. In a relaxed atmosphere, after a brief examination of your eye area, Dr. Nichlos explains exactly how he removes the bothersome under-eye bags, which methods he uses and how the operation can change your appearance for the better.
Every lift is planned individually and precisely matched to your face and eye shape, so that the result looks very harmonious and natural. You do not have to worry that your characteristic facial expression will be changed by the surgery.
At the end of the preliminary consultation, Dr. Nichlos informs you about possible risks and how you need to prepare for the appointment. This includes stopping blood-thinning medication (2 weeks before the surgery, in consultation with your treating physician) and avoiding alcohol and nicotine in order to prevent wound healing problems.
Lower eyelid lift Frankfurt: How does the procedure work?
On the day of the operation, you come to our affiliated clinic. Dr. med. Nichlos and his team welcome you in a state-of-the-art operating room equipped with all the technical refinements of plastic and aesthetic medicine, which allows the highest possible quality of treatment.
You should plan on 1.5 to 2 hours for the lower eyelid lift. As a rule, Dr. Nichlos performs the eyelid surgery under local anesthesia, which is very gentle for you on the one hand and does not require a longer stay on the other. Once the area under the eyes is numbed and Dr. Nichlos has marked the under-eye bags or excess skin to be removed, he makes a short incision along the eyelid contour with a scalpel (external approach). Alternatively, an approach from the inside through the conjunctiva is possible (transconjunctival approach), which has the advantage of usually leaving inconspicuous scars.
For a pure skin lift, excess skin is removed first and the remaining tissue is then tightened. When correcting under-eye bags, prolapsed fatty tissue is also reduced or redistributed.
The plastic and aesthetic surgeon then closes the surgical wounds with fine stitches and covers the treated area with an adhesive dressing. You can then leave the clinic right away. However, we recommend that you do not drive yourself but bring someone with you, as swelling from the surgery can restrict your field of vision.
Healing after the lower eyelid lift: What you should keep in mind
You should get plenty of rest in the first few days and, if possible, sleep with your upper body elevated. Please make sure not to touch or rub the area around the eyes. At first, the area below the lashes will be swollen from the surgery and bruises may form. Cooling can effectively counteract both.
You can go back to work just a few days after the lower eyelid surgery. You are fully back to social life after 7 to 10 days. After the lower eyelid correction, you should avoid sports and other strenuous activities for at least 2 weeks. The same applies to applying makeup, as the fine particles can easily get into the wounds and cause irritation.
About 1 week after the treatment, you come to our practice for suture removal and the first follow-up check. Here, Dr. Nichlos looks at how the healing is progressing and gives you tips on how to best care for the area in the coming months and thus support healing.
What are the risks of a lower eyelid lift?
Removing under-eye bags and excess skin on the lower eyelid is one of the most frequently performed procedures in plastic and aesthetic surgery and, when performed properly by an experienced specialist such as Dr. Nichlos, involves low risks.
As with any operation, lifting the lower eyelid skin can lead to secondary bleeding, infections or wound healing problems. By adhering to strict hygiene and quality standards, however, the risk of these complications can be reduced to a minimum.
What cannot be prevented, owing to the nature of the treatment, are temporary swelling and bruising under the eyes. In addition, in some cases sensory disturbances, slight dry eyes or increased tearing can occur. Side effects of this kind usually subside on their own within a few days to weeks.
How much does a lower eyelid lift cost in Frankfurt?
What is included in the price?
The total cost of a lower eyelid lift is made up of several components. These usually include:
- detailed consultation (€100, fully credited if you go ahead with the treatment)
- individual treatment planning
- surgeon’s fee for the procedure
- medical care during the treatment
- aftercare and follow-up appointments
In addition, anesthesia costs and clinic fees apply, as the procedure is performed in a specialized clinic.
Many patients want to know before treatment what costs are associated with a lower eyelid lift. Since every eye area is individual, the exact price can vary depending on the findings and the extent of the procedure.
At our practice in Frankfurt, the cost of a lower eyelid lift generally starts at:
- Lower eyelid lift: from €3,000
The exact cost depends, among other things, on how pronounced the under-eye bags are and whether excess fatty tissue also needs to be removed or redistributed. After a personal examination, Dr. med. Nichlos prepares an individual treatment and cost plan in which all services are listed transparently.

Frequently asked questions about the lower eyelid lift in Frankfurt
From what age does a lower eyelid lift make sense?
A lower eyelid lift makes sense when under-eye bags, excess skin or wrinkles in the lower eye area bother you, regardless of age. In many cases, patients choose the procedure from the age of 40. However, a correction can also make sense at an earlier age, depending on how pronounced the findings are.
Is a lower eyelid lift painful?
Thanks to the local anesthesia, the procedure itself is not felt as painful. You may feel only the injection of the local anesthetic for a few seconds. During the operation, you may notice a feeling of pressure or a slight tugging. After the procedure, the effect of the local anesthetic lasts for about 2 to 3 hours. If pain occurs after that, you can relieve it well with mild pain medication such as paracetamol.
Can under-eye bags be removed with a lower eyelid lift?
Yes, a lower eyelid lift can remove under-eye bags and excess skin on the lower eyelid, which can make the eye area look fresher.
Are visible scars to be expected after a lower eyelid lift?
Scars cannot be completely avoided in every case with lower eyelid surgery. With a precise incision directly along the lash line, however, Dr. med. Nichlos can reduce their visibility. Or, where possible, he chooses the transconjunctival approach from the inside through the conjunctiva, which usually leaves inconspicuous scars.
With good scar care, the scars from the lower eyelid correction continue to fade in the following months, so that they usually become inconspicuous. In the consultation, Dr. Nichlos explains what you need to pay attention to when caring for the scars in order to achieve an optimal result.
When is the final result of the lower eyelid lift visible?
After the operation, swelling and bruising can develop on the lower eyelid. In our experience, these subside within 4 to 5 days, so that you then get a first impression of the eyelid lift. It takes about two to three months, however, until the scars have fully healed and the last swelling has gone down. At the latest by then, you can see the final result of your lower eyelid correction in the mirror.
How long do the results of a lower eyelid lift in Frankfurt last?
The result of a lower eyelid lift in Frankfurt is generally long-lasting, because the excess skin and fatty tissue under the eyes are removed. Nevertheless, as the skin continues to age, it can loosen again in the eye area, causing under-eye bags to reappear. If you wish, these can be removed with another lower eyelid procedure. You can help maintain the result yourself by protecting your eyes from UV radiation, eating a healthy diet, avoiding smoking and keeping your skin well moisturized.
When does health insurance cover the cost of a lower eyelid lift?
For purely aesthetic reasons, health insurance does not cover the cost. A medical indication may exist if sagging eyelids or under-eye bags restrict your field of vision. It is best to clarify with your insurer before the procedure whether it will pay. Dr. Nichlos runs a private practice without statutory health insurance accreditation: he treats self-pay patients regardless of the indication, and privately insured patients when there is a medical indication.
Can I wear contact lenses or glasses again without problems after the procedure?
After a lower eyelid lift, you should avoid wearing contact lenses for a few days so as not to interfere with the healing of the eye area. Glasses can usually be worn again sooner, as long as they do not put direct pressure on the treated area. Your treating specialist will advise you individually on this.
Is a lower eyelid lift worth it?
That depends on what bothers you. A lower eyelid lift is an option if under-eye bags, excess skin or sagging skin under the eyes bother you and you want a fresher look. If dark discoloration bothers you most, a fat transfer with nanofat may make sense, which is intended to visibly reduce discoloration. Cost (from €3,000 plus clinic and, if applicable, anesthesia) and recovery time are also part of the consideration, and Dr. Nichlos discusses both with you beforehand.
Which lower eyelid lift method is the best?
There is no single best method, it depends on your findings. With an incision along the lash line, excess skin and fat can be removed and, if needed, the muscle can be tightened. The approach through the conjunctiva leaves no external scars and is suitable for patients with under-eye bags but without significant excess skin. Which approach suits you is decided by Dr. Nichlos after the examination.
Can under-eye bags also be removed without surgery?
Excess skin and prolapsed fatty tissue can only be removed surgically. If the under-eye area lacks volume, Dr. Nichlos can fill the tear trough with hyaluronic acid or with your own fat, and fine expression lines around the eyes can be smoothed with botulinum toxin. This does not replace the lift, but complements it. You can read more about the causes in our guide Under-eye bags: where they come from and what helps.
