Firming sagging breasts after pregnancy or weight loss with a mastopexy
Many women want a beautiful décolleté with firm breasts. With advancing age, pregnancy and breastfeeding, or significant weight loss, however, breasts that were once full and perky can increasingly lose shape and volume and sag. The result is sagging breasts, which many women find unattractive and therefore want to change.
With the breast lift, plastic surgery offers the possibility of raising and reshaping loose breasts to give them back their feminine and youthful look. For an all-round harmonious result, the nipples are also repositioned higher.
Reshaping the breast and adapting it to the body proportions, however, requires a great deal of experience and a very fine, well-developed sense of aesthetics. Dr. Nichlos brings these qualifications as a specialist in plastic surgery and routinely performs the procedure at the affiliated clinic in Frankfurt. Schedule an appointment for a consultation now!
What is a breast lift?
A breast lift (also called mastopexy) is a surgical procedure to firm and improve the shape of the female breast. The aim of this procedure, which is frequently performed in breast surgery, is to raise and visibly firm breasts that have loosened due to age, pregnancy or weight reduction. To do this, the excess breast skin is surgically removed and, if necessary, the nipple is repositioned higher.
However, a pure breast lift only corrects the shape and does not add breast volume. On the other hand, no volume is lost either. If desired, the breast lift can be combined with a breast augmentation or with a breast reduction. In addition, the breast lift is often part of a mommy makeover. Let Dr. med. Nichlos advise you on this in a personal conversation at our practice in Frankfurt.

Experiences with the breast lift at Dr. Nichlos
What are the benefits of a breast lift in Frankfurt?

Specialist for breast lift in Frankfurt
Dr. med. Emmanouil Nichlos has specialized in breast lifts for over 17 years and has extensive experience in aesthetic breast surgery.
With precise technique and a strong sense of natural shapes, Dr. Nichlos restores a firm and harmonious breast shape. Dr. Nichlos personally looks after freshly operated patients. In the first 24 hours after the operation, he is available to them for questions or problems.
What are the reasons for a breast lift?
Over time, every body changes, and natural aging processes cause the skin and connective tissue to loosen more and more. Unfortunately, this also shows in the breasts. After major weight loss or a pregnancy with a long breastfeeding period, too, breasts that were once full can lose a lot of volume and begin to sag. Many women cannot and do not want to accept this breast shape, somewhat unkindly called sagging breasts.
From experience, we know that dissatisfaction with one’s own breasts and a strong wish for a youthful, firm breast shape can cause great distress in women and trigger feelings of shame or even problems in relationships. With a breast lift, modern plastic and aesthetic surgery can contribute to more self-confidence and a sense of well-being in your own body.

When is the right time for a breast lift?
A breast lift is generally possible at any age. What matters is that the breast is fully developed. This is usually the case between the ages of 18 and 21. The right time for the lift is determined above all by your personal level of distress. Does your breast bother you a lot in its current form, or does it even restrict you in your everyday life? Only you can answer that. There are also other factors you should take into account when planning.
If your family planning is not yet complete, Dr. Nichlos generally advises waiting with the procedure. During pregnancy and breastfeeding, your breast first increases in volume and then decreases again. In the worst case, the operation would then have been in vain. In rare cases, a breast lift can also limit the ability to breastfeed. Ideally, your last breastfeeding period was at least six months ago. This ensures that the breast has fully regressed.
Your body weight also plays an important role in planning. Major weight fluctuations up or down inevitably affect the size and shape of your breast as well. If you are planning a larger weight loss, it makes sense to wait with the procedure until you have reached your target weight and kept it stable for several months. After that, nothing stands in the way of a lift.
Before any breast procedure, Dr. Nichlos also recommends an ultrasound examination or, alternatively, a mammogram of the breast. The purpose is to rule out abnormalities in the breast tissue, and it is part of the careful surgical preparation at our practice.
Which surgical techniques are used for the breast lift?
The various surgical techniques differ mainly in their incision pattern. Which incision technique Dr. med. Nichlos uses in breast surgery depends on the basic shape of the breast, but above all on the amount of excess skin that has to be removed for the desired result. The individual incision techniques are named after letters that resemble the shape of the incision:
In the so-called O technique or donut technique (periareolar lift), the surgeon makes the skin incision in a circle around the nipple. It is the simplest variant of the breast lift and leaves a scar that usually heals inconspicuously.
The I technique (vertical lift) is used very frequently in plastic surgery. In this technique, the specialist makes the O incision and, in addition, a vertical incision from the nipple downward. Based on the incision pattern, this technique is also called the lollipop technique. It is suitable for operations in which larger amounts of excess skin have to be removed.
With a very large amount of excess skin, the T technique (also T incision) is a good option, in which the surgeon additionally makes an incision in the inframammary fold. Together with the circular incision around the nipple and the vertical incision to the inframammary fold, this incision pattern looks like an anchor, which is why it is also called an anchor incision. In some cases, only a horizontal incision along the breast fold to the side is made, which is then called an L incision.
What types of breast lift are there and how can they be combined?
Which type of breast lift is right for you depends on what exactly bothers you about your breasts. For many patients, the shape alone is not the problem. Some women also want more volume, others less. Dr. Nichlos discusses the options with you in detail and recommends the method that best suits your wishes and your starting situation.
Breast lift alone
In a breast lift alone, Dr. Nichlos firms the loose skin and the mammary gland. The volume remains unchanged, so your breast is almost exactly as large as before the procedure. This method is suitable if you are happy with the size of your breasts and only want to have their shape corrected.
Breast lift with breast augmentation using implants
If your breast has not only become looser after a pregnancy or major weight loss but also feels downright empty, it is possible to combine the lift with an augmentation using implants. This gives your breast fullness again.
Breast lift with breast augmentation with fat transfer
If you want more volume in your breast but do not want silicone implants, breast augmentation with fat transfer may be an alternative. Dr. Nichlos takes fat cells, for example from the abdomen or thighs, processes them and places them in the breast. As a rule, this can gain half to one cup size.
Breast lift with breast reduction
Some women have breasts that are both large and very loose. Here, a breast lift alone is often not enough to achieve a satisfying result. In addition, the weight of the breast would lead to sagging again in the long run. In a combination with a breast reduction, Dr. Nichlos removes excess fatty and glandular tissue and tightens the skin envelope.
For each of these different methods of breast lift, we have different surgical techniques available.
What happens before the breast lift?
So that your breast lift can be carried out smoothly, a prior physical examination and a personal consultation are required. During the examination, Dr. med. Nichlos checks whether you meet all the health requirements for the procedure and whether there are any medical reasons against an operation on your breast.
Together you discuss which form of breast lift (with or without a breast augmentation or reduction) is best suited to fulfill your wishes. Dr. Nichlos then explains the exact course of the operation, what result you can expect and what you should pay attention to before and afterward to support the breast lift as well as possible.
Important: To lower the risk of complications and to promote wound healing after the operation, you should stop smoking and avoid alcohol about 2 weeks before the operation. If you take blood-thinning medication, you should stop it temporarily, in consultation with Dr. Nichlos.
Breast lift in Frankfurt: How does the surgery work?
We perform the breast lift under general anesthesia at our affiliated private clinic in Frankfurt. The operation takes about 1.5 to 2.5 hours. After the anesthesia is induced and the surgical area has been thoroughly disinfected, Dr. Nichlos makes the necessary incisions, depending on the chosen surgical technique, repositions the nipple and, if necessary, also reduces the areola.
In the next step, the specialist removes the excess skin and reshapes the remaining tissue with the mammary gland so that your breast gets a beautiful, round and firm shape. Finally, the wound is closed with self-dissolving sutures.
For stabilization, you receive a dressing and a support bra/compression bra, which you wear for 6 weeks. For optimal healing and a stable surgical result, it is very important that you reliably follow our aftercare instructions.
Healing after the breast lift: What to keep in mind
After the breast lift, you should take it easy physically. To ease the initial swelling, redness and tightness, you can cool the breast gently, but not directly on the skin. For 6 weeks you wear a special support bra that supports healing and the breast and stabilizes the result. You avoid sports as well as heavy lifting and carrying for 4 to 6 weeks. Sleep on your back if possible. A drain is not necessary, and the next day you are usually socially presentable again. Lymphatic drainage and follow-up appointments take place as agreed.
Important: As soon as the wound has closed, you can begin scar care. Also make sure to avoid pulling on the scar as far as possible, and protect it from direct sunlight for the next 6 to 12 months to avoid hyperpigmentation.
What are the risks of a breast lift?
As a surgical procedure, a breast lift involves certain general surgical risks, above all bleeding and secondary bleeding, wound healing disorders and infections. These risks can be reduced if the procedure is performed under strict hygiene conditions by an experienced specialist in plastic and aesthetic surgery such as Dr. Nichlos.
The same applies to the specific risks of a breast lift, such as the (mostly temporary) loss of sensation in the skin or nipple, asymmetrical or uneven results, or conspicuous scarring. Dr. Nichlos informs you comprehensively about possible risks and complications before the procedure and will of course be happy to answer all your questions about the breast surgery.
How much does a breast lift cost in Frankfurt?
What is included in the price?
The total cost of a breast lift is made up of several components. These usually include:
- detailed consultation (€100, fully credited if you go ahead with the treatment)
- individual treatment planning
- surgeon’s fee for the operation
- medical care during the treatment
- compression bra for the healing phase
- aftercare and follow-up appointments
In addition, a breast lift involves anesthesia costs and clinic fees, as the procedure is performed under general anesthesia in a specialized clinic. If an overnight stay in the clinic is planned, depending on your individual course or at your personal request, the cost of that is added.
Many patients want to know before treatment what costs are associated with a breast lift. Since every treatment is planned individually, the exact price can vary depending on the initial findings, the extent of the operation and the method chosen.
At our practice in Frankfurt, the cost of a breast lift generally starts at:
- Breast lift: from €7,500
The exact cost depends, among other things, on how pronounced the skin laxity is and which surgical technique is used to achieve a harmonious result. After a personal examination, Dr. med. Nichlos prepares an individual treatment and cost plan in which all services are listed transparently.

Frequently asked questions about the breast lift in Frankfurt
Will visible scars remain?
Scars can never be completely avoided in a surgical procedure. Thanks to modern surgical techniques and careful incision placement, they usually heal inconspicuously. Consistent scar care can further improve their appearance.
How long do the results of a breast lift last?
The result of a breast lift is generally long-lasting. However, even surgery cannot stop the natural aging process. Over time, skin and breast tissue loosen, so the shape of the breast changes again. Pregnancy or major weight fluctuations can also affect the result.
Can I still breastfeed after a breast lift?
Yes, this is usually possible. In a breast lift, excess skin is removed and the breast tissue is reshaped, while the milk ducts remain untouched. In rare cases, the ability to breastfeed can still be limited. In addition, a pregnancy can change the shape of the breast and thus the result. We therefore recommend waiting with the lift until your last breastfeeding period was at least six months ago and your family planning is complete.
Does the nipple remain sensitive after a breast lift?
The sensitivity of the breast and nipple can be temporarily altered after the procedure. In most cases it normalizes after a few weeks or months. As with any operation, a permanent change cannot be ruled out. Dr. Nichlos will discuss this with you beforehand.
Can the breast be lifted more than once?
A second lift is generally possible if skin and tissue have changed again after a pregnancy or weight changes. Whether this makes sense depends on the condition of your skin and the first operation. You will clarify this in the consultation with Dr. Nichlos.
Is a breast lift painful?
The operation takes place under general anesthesia, so you feel nothing during it. Afterward, wound pain and a feeling of tightness can occur. They can usually be treated well with pain medication.
What is the difference between a breast lift and a breast reduction?
In a breast lift, excess skin is removed and the breast is raised, while the volume remains largely the same. In a breast reduction, glandular and fatty tissue is also removed, making the breast smaller and lighter. Both procedures can be combined.
