Removing stubborn fat deposits with liposuction at Dr. Nichlos' practice

Do you watch your diet closely, exercise a lot and still have stubborn fat deposits that simply will not go away? Especially in the typical problem areas such as the abdomen, hips, buttocks and legs, this is unfortunately not unusual. With liposuction in Frankfurt, we can support you on your way to your desired figure.

Liposuction is one of the most common procedures in plastic and aesthetic surgery and is generally performed on an outpatient basis. During the treatment, Dr. Nichlos loosens the resistant fatty tissue and then gently suctions it out. Once removed, fat cells do not come back.

Dr. Nichlos will gladly explain in detail in a personal consultation whether liposuction is an option for you and which method is suitable. To do so, book a consultation at our practice in Frankfurt by phone or use our form to request an appointment.

What is liposuction?

Liposuction (also called lipoplasty) is a procedure in plastic and aesthetic surgery in which fatty tissue is surgically removed. In most cases, liposuction is performed for aesthetic reasons, to remove unwanted fat accumulations and define the body silhouette. In some cases, however, medical reasons such as lipedema (a pathological disorder of fat distribution) make the removal of fat cells necessary.

Specialist for liposuction in Frankfurt

Dr. med. Emmanouil Nichlos has specialized in liposuction for more than 17 years and has extensive experience in aesthetic body contouring.

Using modern techniques, Dr. Nichlos specifically removes unwanted fat deposits and shapes a defined, harmonious silhouette. Dr. Nichlos personally looks after patients who have just had surgery. In the first 24 hours after the operation, he is available to them for questions or problems.

Who is liposuction suitable for?

Liposuction is an option for patients who have fat deposits that persist despite diets and physical exercise and make their figure look unflattering. Fat deposits can occur in different body regions.

Liposuction can be performed on the classic problem areas such as the abdomen, hips or buttocks, as well as on the neck, chin or the male breast. A common indication is lipedema, which occurs mainly in women and rarely in men. In this condition, fatty tissue multiplies uncontrollably on the arms and legs, is often very sensitive to pain and pressure, and severely limits the quality of life of those affected. Since conservative treatment can at best delay progression, liposuction offers a way to remove the fat deposits and relieve the symptoms.

Liposuction is not suitable for pure weight loss. You should already have reached your comfortable weight before liposuction and have kept it successfully for some time. With the help of liposuction, Dr. Nichlos can then remove the last unwanted fat deposits and optimize the contours of your body so that you feel completely comfortable again.

Body areas

Which body areas can be treated with liposuction?

Dr. Nichlos removes fat where it persists despite exercise and diet. These are the areas treated most often.

Abdomen and hips

Fat deposits on the abdomen and hips are among the most common concerns. If the skin has become loose after pregnancy or significant weight loss, liposuction alone is often not enough. In that case, a tummy tuck can be a sensible option.

Thighs and knees

On the thighs and knees, individual deposits can be shaped in a targeted way. Lipedema, a pathological increase in fat on the legs and arms, follows different rules. You can find more on the lipedema page.

Upper arms

Fat can also be removed from the upper arms. If the skin sags afterwards, an arm lift may be an option.

Double chin and neck

Fat can also be removed from the chin and neck. You can find all the information on this under double chin removal.

Back and male breast

Fat can also be removed from the back. In men, liposuction is also part of the treatment of gynecomastia.

Methods

Which methods are there for liposuction?

Since the beginnings of liposuction in the mid-1970s, the methods have continued to evolve, so that several surgical techniques are available today. They differ in approach and offer different advantages:

Tumescent liposuction (TAL method)

The standard method and the most frequently used technique for liposuction is tumescent-assisted liposuction (TAL for short). A larger amount of a so-called tumescent solution is first injected into the treatment area. It has a numbing effect and also loosens the fat cells so that they can then be detached and suctioned out more easily. TAL allows larger amounts of fat to be removed.

Vibration liposuction (VAL or PAL method)

In vibration-assisted liposuction (VAL for short, sometimes also called PAL, power-assisted liposuction), vibrating cannulas are used during the procedure. They make it easier to detach the fat cells so that they can be suctioned out evenly. This gentle method is very well suited to precise body contouring, even in difficult areas such as the knee or back, and is also used for fat removal in lipedema treatment.

Laser lipolysis (LAL method)

Fast and gentle laser lipolysis, or laser-assisted liposuction (LAL for short), also uses a tumescent solution to loosen the fat cells, but then uses a medical laser in addition to the suction cannula. This laser works with two wavelengths: one liquefies the fat cells, and the other tightens the skin and stimulates the body’s own regeneration and collagen production. This can improve the appearance of the skin and achieve a good tightening effect, which, when smaller fat deposits are removed, can make additional tightening of the abdomen, thigh or upper arm unnecessary.

Ultrasound-assisted liposuction (UAL method)

With this gentle technique as well, the fat cells are liquefied beforehand. Ultrasound is used to rupture the fat cells before they are suctioned out with a conventional cannula. This method also allows precise contouring of the treatment areas. It is also very well suited to treating firmer regions (e.g. the back). With a small amount of excess skin, it can already achieve a tightening effect on the skin, as the strong heat of the ultrasound causes the skin to shrink somewhat.

Water jet-assisted liposuction (WAL method)

In water jet-assisted liposuction (WAL for short), a device that sprays a fan-shaped water jet into the fatty tissue is inserted together with the suction cannula. The jet gently releases the fat cells from the tissue, and they are suctioned out directly through the cannula. For smaller fat excesses, the treatment can be performed without general anesthesia.

What happens before liposuction?

Before the treatment, we invite you to a detailed consultation with Dr. Nichlos at our practice in Frankfurt. In this conversation, held in a relaxed atmosphere, the specialist in plastic surgery first wants to learn more about you and the wishes and goals you associate with liposuction. Dr. Nichlos will also carry out an initial physical examination of the areas with unwanted fat accumulations.

If liposuction is an option for you, Dr. med. Nichlos will explain the course of the treatment in detail and inform you about the associated risks. You are of course welcome to ask questions about the procedure at any time, and the specialist will gladly answer them in detail.

To prevent complications during and after the treatment, it is important that you inform us in advance about any allergies, intolerances and pre-existing conditions and, if you have one, provide a medication plan. Blood-thinning medication in particular is problematic and should be paused temporarily before liposuction (in consultation with the doctor).

Procedure

Liposuction in Frankfurt: How does the surgery work?

For liposuction, you come to our affiliated private clinic in Frankfurt. Here Dr. Nichlos performs the liposuction in a technically state-of-the-art operating room. Depending on the indication, the operation is performed on an outpatient basis or with a short hospital stay, under general anesthesia or, for smaller amounts, under tumescent anesthesia with twilight sedation. It takes about 0.5 to 3 hours, depending on the extent.

Before the procedure, Dr. Nichlos marks the areas with the fat deposits to be removed. As soon as the anesthesia takes effect and you are asleep, the specialist injects a so-called tumescent solution into these areas. This causes the fatty tissue to separate from the surrounding tissue and makes the fat cells easier to suction out. At the same time, the tumescent solution reduces bleeding and has a numbing effect, so that general anesthesia is not always necessary for smaller liposuction procedures.

After a certain waiting period (approx. 30 to 40 minutes), Dr. Nichlos makes small skin incisions with a scalpel, through which he inserts fine cannulas into the fat deposits. With gentle movements, he loosens the fatty tissue and suctions out the excess fat cells until the body contour is shaped as desired. He then sutures the small skin incisions or uses special plasters. Finally, he applies a bandage or a compression garment to reduce swelling.

Aftercare

Healing after liposuction: What you should keep in mind

After liposuction, you wear the bandage or compression garment for 4 to 6 weeks. For the first 10 days after your liposuction, please wear it continuously so that the pressure on the treated area stays constant, no fluid can collect and swelling subsides better.

To support the healing process, you should also arrange an appointment for lymphatic drainage with a physiotherapist or masseur early on (ideally a few days after the surgery). The gentle massage carries excess fluid and toxins away from the tissue and relieves pain. It also helps prevent seromas (fluid collections). In the first weeks after the surgery, 1 to 2 sessions per week are recommended.

Please also remember to take it easy physically in the first period after the surgery. If possible, avoid strenuous housework and refrain from heavy exertion. You are generally able to work and back to social activities after about 7 to 10 days. Avoid sports for 4 to 6 weeks. The sutures are removed after about 1 week.

About a month after your liposuction, the swelling from the operation has subsided enough that the preliminary result of the treatment can be assessed at the follow-up appointment. After about a year, the final result of the liposuction is visible.

Risks

What are the risks of liposuction?

As a surgical procedure, liposuction carries general surgical risks, including secondary bleeding, wound healing disorders and infections. However, these can be reduced by proper performance by an experienced specialist such as Dr. Nichlos and by adhering to strict hygiene guidelines at our practice in Frankfurt. Nevertheless, side effects can occur in rare cases, such as temporary feelings of tension or numbness in the treated area. In our experience, these subside again within a short time, as do redness, bruising or swelling at the treated sites. Despite the greatest care, it cannot always be ruled out that slightly irregular contours or small dents in the skin are visible after the operation.

FAQ

Frequently asked questions about liposuction in Frankfurt

For purely aesthetic liposuction, health insurance generally does not cover the cost. Only in exceptional cases, such as documented stage 3 lipedema, may coverage of costs be possible. However, Dr. Nichlos does not hold statutory health insurance accreditation. Please clarify any possible subsidies directly with your health insurer.

The fat cells that are removed do not grow back. However, about 30% of the fat cells remain in the treated area and can grow again if you gain a significant amount of weight. With an active lifestyle, the result can be kept stable in the long term.

Whether the skin retracts after liposuction depends on factors such as skin elasticity, age and genetic predisposition. In some cases, additional skin tightening or a complementary laser treatment can be useful. Dr. Nichlos will advise you individually in a personal consultation.

Yes, one possible alternative is injection lipolysis (also called the “fat-dissolving injection”), which can reduce smaller fat deposits without surgery. However, this method is only suitable for limited areas such as the chin or the flanks. Dr. Nichlos will gladly discuss with you in a personal consultation whether injection lipolysis makes sense for you.

As a rule, up to 5 liters of fat can be removed per treatment. The exact amount depends on your individual starting findings and your general health.

Light everyday activities are usually possible again after a few days. Strenuous physical activity should be avoided for about 4 to 6 weeks. Wearing a compression garment supports the healing process and improves the result.

Thanks to the anesthesia, you will not feel anything during the procedure. Afterwards, you may experience discomfort similar to sore muscles and mild swelling, which can be treated well with pain medication.

The preliminary result can be assessed after about one month and is usually largely visible after 3 to 6 months. The final result shows after about a year, once the swelling has fully subsided and the skin has adapted to the new body shape.
