Tightening loose skin & abdominal muscles through abdominoplasty with Dr. Nichlos

Exercise, changes in diet, weight loss injections: people try many things to get a beautiful, flat stomach. The disappointment is all the greater when the abdominal skin sags after substantial weight loss and forms an unsightly apron of skin. After a pregnancy, too, excess skin sometimes remains on the lower abdomen. More exercise, further diets and other measures often can do little about it.

In many cases, a surgical tummy tuck offers an effective way to remove the apron of skin and bring the abdominal wall back into shape. It removes the excess tissue that has been stretched and has become loose. At the same time, the abdominal wall is tightened so that the silhouette looks attractive and harmonious again.

Do you also suffer from an apron of skin and long for a firm, flat stomach again? Then a specialist in plastic and aesthetic surgery such as Dr. med. Nichlos is the right person to talk to. The surgeon performs the tummy tuck routinely and can help you gain a new sense of your body. Use our online form to request a personal consultation. We will get back to you promptly.

What is a tummy tuck?

In a tummy tuck (also called abdominoplasty), excess fatty tissue and loose skin on the abdomen are surgically removed and the underlying muscle and connective tissue are tightened to achieve an aesthetic, flat midsection.

Excess skin and fatty tissue on the abdomen develop, for example, after substantial weight loss or when the skin does not fully retract after a pregnancy. A tummy tuck in Frankfurt can also make sense for age-related skin laxity or weak connective tissue, in order to improve the body contour.

Reasons for a tummy tuck

Who is a tummy tuck suitable for?

A tummy tuck is a major procedure that deserves careful thought. Dr. Nichlos can best assess whether the surgery makes sense in your case at an appointment in his practice in Frankfurt. Experience shows that the following patient groups benefit most:

After substantial weight loss

You can be proud of substantial weight loss. Unfortunately, the skin often does not retract to the same extent and hangs over the waistband as an apron of fat. This is not just a purely aesthetic problem but can also weigh on your mind. Exercise and dieting cannot change that, but a tummy tuck can. Dr. Nichlos removes the excess and gives your abdomen a smooth contour again that also suits your new weight. The overview of skin tightening after weight loss shows which other body regions come into question after substantial weight loss.

After pregnancies

The baby is finally here, but unfortunately the belly is still there too. This is the case for many women whose skin and tissue and the separated abdominal muscles (diastasis recti) do not want to retract despite discipline and exercise. In a tummy tuck, Dr. Nichlos removes the bothersome excess skin, tightens the muscles and thus restores the stability of the abdominal wall. If the breasts have also changed after pregnancy, a mommy makeover can combine several procedures.

With genetically determined weak connective tissue

Even without major ups and downs on the scale or a pregnancy, the skin in the abdominal area can become loose. In most cases, this is due to an inherited weakness of the connective tissue. As a result, the skin loses elasticity faster and can sag due to gravity. If you wish, Dr. Nichlos removes the excess skin and thus helps you get a flat, firm stomach again.

With functional problems caused by an apron of fat

A pronounced apron of fat is not only an aesthetic problem. Sometimes it also causes serious discomfort in everyday life, ranging from inflammatory skin irritation to fungal infections. The associated pain often makes wearing clothes very uncomfortable. With a tummy tuck, Dr. Nichlos can achieve a significant improvement in these cases.

Types of tummy tuck

What types of tummy tuck are there?

In Frankfurt, Dr. med. Nichlos performs the tummy tuck either as a mini tummy tuck (tightening a specific area of the abdomen) or as a full tummy tuck.

Partial tightening

For partial tightening, Dr. Nichlos distinguishes between three variants depending on the findings:

The mini tummy tuck is suitable for athletic, slim people with slightly excess skin below the navel. Dr. Nichlos removes the excess skin through a small incision in the pubic area. Correcting the muscles or moving the navel is not necessary. The scar is small and usually heals quickly.

If a somewhat larger area of skin is affected, the extended mini tummy tuck is used. Here it may be necessary to move the navel and perform a slight muscle tightening. This variant bridges the gap between the mini tummy tuck and the full tummy tuck.

In the upper abdominal lift (reverse abdominoplasty), the focus is on the area above the navel. Dr. Nichlos tightens the upper abdomen with an upward pull through an incision below the breast fold. Moving the navel is not necessary, and the scar in this location is usually not very noticeable.

Full tummy tuck

If the excess skin extends beyond the lower abdomen or the abdominal muscles have separated significantly (diastasis recti), a full tightening is usually required. For this, Dr. Nichlos detaches the skin over a large area, tightens the muscles and finally moves the navel to its natural position. In very pronounced cases, such as after massive weight loss, the tightening can also be combined with a buttock lift to form a body lift.

Body lift

When we combine the tummy tuck with a buttock lift, we speak of a body lift (circumferential abdominoplasty). This usually requires two sessions in our affiliated private clinic in Frankfurt, with the abdomen tightened in the first and the buttocks in the second. A body lift is an option for people with very pronounced excess skin after significant weight loss.

What incision does Dr. Nichlos use for the tummy tuck?

That depends entirely on the amount of excess skin to be removed and your individual anatomy. Dr. Nichlos always plans the incision so that the scars usually heal inconspicuously and can be concealed well.

Lower abdominal incision

Lower abdominal incision

For a small amount of excess skin on the lower abdomen, Dr. Nichlos places the incision in an arc along the pubic hairline. Through this access, he removes excess skin and tissue and, if needed, tightens the muscles. The scar is later usually covered by underwear or a bikini.

Lower abdominal incision with navel repositioning

Lower abdominal incision with navel repositioning

For a moderate amount of excess skin, Dr. Nichlos additionally makes a round incision around the navel. This allows him to tighten the middle abdominal area as well. The navel itself is not altered and is simply brought out through a new opening in the tightened abdominal wall.

Fleur-de-lis incision

Lower abdominal incision with vertical incision (fleur-de-lis)

For very pronounced excess skin, a vertical incision from the navel downward is added. This makes it possible to tighten the abdominal wall in both directions. The scar is more visible in this case. However, it is the only way to correct very pronounced excess skin.

Combining a tummy tuck and liposuction

In a tummy tuck, Dr. Nichlos removes excess skin and tissue and tightens the muscles. The surgery does not affect the fat deposits underneath. For this reason, the specialist combines the tightening with liposuction in many cases. This allows him not only to smooth the abdominal wall but also to refine the contour around the hips and back. The result is very harmonious and cannot be achieved with the tightening surgery alone.

Dr. Nichlos will gladly discuss with you in a personal conversation whether and to what extent liposuction is possible and advisable in your specific case.

Preparation
Preparation

What happens before the tummy tuck?

Good preparation and planning are the basic prerequisites for an optimal result in a procedure like the tummy tuck. It begins with a preliminary examination at our practice, which consists of a detailed medical history and a physical examination.

In the consultation that follows with Dr. med. Nichlos, the focus is on your wishes and goals and the treatment methods best suited to you. He informs you what result you can expect, explains the exact course of treatment, possible risks and complications as well as the healing process, and tells you what to pay attention to before and after the procedure.

The cost is also discussed during the consultation. As with all procedures in plastic and aesthetic surgery, a flat amount can usually not be given, since every operation is very individual. However, after the consultation you will receive an individual quote in which all costs are itemized in detail.

Procedure

Tummy tuck in Frankfurt: How does the surgery work?

In the affiliated clinic in Frankfurt, we generally perform the tummy tuck under general anesthesia. At the start, Dr. Nichlos marks the course of the incision on your skin, and then the anesthesia is induced. As soon as the anesthesia takes effect, the operation can begin.

Depending on the treatment method, the surgery takes 1.5 to 4 hours, and the sequence can vary somewhat. First, Dr. Nichlos precisely makes the necessary skin incision or incisions and then carefully detaches the skin and fatty tissue from the abdominal wall. If necessary, he tightens the abdominal muscles.

In the next step, the specialist in plastic and aesthetic surgery moves the navel to the optimal position (depending on the surgery). Before suturing, he places drains through which wound fluid can escape and fits you with a compression bandage (also called an abdominal binder). Depending on the extent of the surgery, you must wear it for 4 to 6 weeks.

After the operation, you stay in the clinic for 1 to 2 nights. The drains are removed after about 1 to 3 days. Since we use self-dissolving sutures, our patients do not need suture removal. You can support the healing process and the result by following our aftercare instructions.

Healing after the tummy tuck: What to keep in mind

After the tummy tuck, allow yourself a few days of rest and give your body the chance to recover. During this time, be sure to wear the recommended compression garment. It helps initial swelling subside faster and stabilizes the result of the surgery. Immediately after the tummy tuck, temporary numbness in the abdomen may still occur, but it goes away on its own.

After 10 to 14 days, you are usually back to social life. Depending on the extent of the procedure, however, it can take different lengths of time until your full capacity is restored. Avoid heavy lifting and sports that strain the abdominal muscles for 4 to 6 weeks.

The initially reddened scars fade more and more over time. To avoid hyperpigmentation, protect them from direct sunlight for the first 6 months and use good scar care products. We are happy to advise you on this.

Be sure to attend the follow-up appointments so that Dr. Nichlos can monitor the healing process, detect any complications early and, if necessary, take countermeasures. Of course, he is also available to you after the surgery for any questions.

Risks

What are the risks of a tummy tuck?

A tummy tuck is a surgical procedure and, as such, comes with certain risks and possible complications. These include above all general surgical risks such as postoperative bleeding, wound healing disorders and infections.

Thanks to the extensive expertise and many years of experience of Dr. med. Nichlos and the professional performance of the procedure under strict hygiene standards, these risks can be reduced, but they cannot be ruled out entirely.

Nevertheless, temporary redness, swelling or sensory disturbances can occur in the surgical area. These usually subside on their own in the course of healing. In very rare cases, a protruding belly can develop as a complication. In this case, the tightened abdomen bulges forward. The reasons vary and range from fluid accumulation to individual anatomical conditions.

Of course, Dr. Nichlos will inform you comprehensively about all possible risks and complications before the surgery.

FAQ

Frequently asked questions
about the tummy tuck in Frankfurt

The result of a tummy tuck generally lasts for the long term. However, it can change again through substantial weight gain, a pregnancy or the natural aging of the skin.

The mini abdominoplasty affects only the area below the navel and is less extensive. It is suitable for slightly excess skin or mild muscle laxity. The full tummy tuck affects the entire abdomen, tightens the muscles if needed and moves the navel. It is used for more pronounced findings.

Yes, since this procedure removes excess skin and the remaining skin is tightened and sutured, scars are unfortunately unavoidable. As a rule, this is a horizontal scar that runs along the pubic area so that it is covered by underwear. If the navel also has to be moved, a fine circular scar remains around the navel. In very large procedures, an additional vertical incision may be required, which also forms a scar.

With his great experience as a surgeon and the use of gentle methods, Dr. med. Nichlos always strives to keep scars as small as possible for patients. Over time, they mostly fade and match the color of the surrounding skin. Good scar care can support this process.

Pregnancy and childbirth are a peak performance for the female body, so you should give it time to recover from the strain afterward. The tissue usually needs several months to retract, although this often does not happen completely.

After about half a year, however, it is possible to estimate which changes will probably remain permanent and whether they can be corrected with a tummy tuck in Frankfurt. So that the result is generally lasting, it is advisable that you have already reached your desired weight before the procedure and have kept it for a while. For this reason, we recommend waiting 9 to 12 months after giving birth for a tummy tuck.

In principle, another pregnancy is possible after a tummy tuck. However, the extreme stretching of the abdominal skin during pregnancy changes the result of the procedure. For this reason, we recommend having a tummy tuck only once your family planning is complete.

As a rule, this is an aesthetic operation that is not covered by health insurance. Since Dr. med. Nichlos does not have statutory health insurance accreditation, treatment is provided exclusively on a private basis.

The operation removes excess skin and tightens the muscles, but it does not affect the fat deposits underneath. For this reason, Dr. Nichlos combines the tightening with liposuction in many cases. In addition, initial swelling and, in very rare cases, a protruding belly can affect the shape of the abdomen. Dr. Nichlos will assess what is behind it in your case during the examination and at the follow-up appointments.

If the excess skin mainly affects the abdomen, the tummy tuck is the first option, whether after weight loss or after a pregnancy. If several body regions are affected after substantial weight loss, you will find the overview on the page Skin tightening after weight loss. If the breasts have also changed after a pregnancy, Dr. Nichlos combines several procedures into a mommy makeover.
