Reducing excessive sweating long term: sweat gland suction with Dr. Nichlos
Sweating is a completely normal and even vital function of our body, and its purpose is to protect our organism from overheating during physical exertion or heat. In hyperhidrosis, this function is disrupted, which means that those affected sweat not only during sports or at high temperatures, but also experience excessive and uncontrollable sweating on the hands, feet and under the arms at rest and in cool temperatures.
For those affected, the seemingly causeless and excessive sweating is a heavy social and emotional burden. Visibly soaked clothing, the odor associated with sweating, or suddenly sweaty palms can put them in situations that are very uncomfortable for them and trigger a strong sense of shame.
Especially with hyperhidrosis axillaris (hyperhidrosis in the underarms), conventional measures such as deodorant usually bring no relief, so suctioning out the sweat glands in the underarm area is one way to reduce the sweat production that has gotten out of control. As a plastic and aesthetic surgeon, Dr. Nichlos regularly performs the procedure on the sweat glands in Frankfurt and is regarded as an expert in the field. What is sweat gland suction?
In sweat gland suction, the sweat glands responsible for excessive sweat production in the underarm area are surgically removed for the most part (up to 98 percent). In principle, the procedure is similar to liposuction.
A tumescent solution is injected into the area to be treated through small skin incisions. On the one hand, it has a local numbing effect and reduces bleeding, and on the other hand it makes the subsequent removal of the sweat glands with fine cannulas easier. The minimally invasive procedure can be performed on an outpatient basis. A hospital stay is usually not necessary.

Who does sweat gland suction make sense for?
Sweat gland suction (also suction curettage) makes sense for people who suffer from severe hyperhidrosis (pathological sweating) in the underarms that cannot be controlled with conventional means and therefore affects their quality of life.
Typical of the pathological overactivity of the sweat glands is that those affected sweat heavily regardless of physical exertion such as sports or temperature. The sweating often comes in bursts and is practically unpredictable, so it can also occur suddenly in contact with other people and cause a sweaty forehead, wet hands or damp underarms. For many patients who visit our practice in Frankfurt, this creates a strong sense of shame. In extreme cases, they even avoid contact with other people out of fear of sweating episodes. At the latest then, a treatment such as sweat gland suction is urgently advisable.

Are there alternative treatments to sweat gland suction?
Besides sweat gland removal, hyperhidrosis can also be treated, for example, by injecting Botox. By specifically blocking the transmission of nerve impulses, the active ingredient can reduce sweating in both amount and frequency. In contrast to suction, Botox treatment can also be used not only in the underarms but in other areas affected by hyperhidrosis such as the hands or feet.
One disadvantage, however, is that Botox reduces sweat production only temporarily (usually for 3 to 6 months). After that, repeat treatment is necessary. With repeated treatments with botulinum toxin, however, the effect can gradually last longer. Get advice from Dr. med. Nichlos in Frankfurt now on which method (sweat gland suction or Botox injection) is better for you to get your symptoms under control for as long as possible.

What happens before sweat gland suction?
Beforehand, Dr. Nichlos welcomes you to a detailed consultation at our practice in Frankfurt. Here, the specialist in plastic and aesthetic surgery performs a physical examination and asks you questions about the frequency and intensity of your problem. For example, he will ask in which situations you sweat excessively (only with or also without physical exertion).
Once Dr. Nichlos has gotten an impression of the current situation, he will present the treatment that is optimal for you. For predominantly axillary hyperhidrosis (sweating in the underarms), this is surgical sweat gland suction.
Dr. Nichlos will explain the suction procedure to you, inform you about possible risks and complications, and give you instructions on how to behave before the surgery (e.g. avoiding alcohol and nicotine, stopping blood-thinning medication).
Sweat gland suction in Frankfurt: How does the surgery work?
On the day of the operation, you come to our affiliated private clinic in Frankfurt for the treatment, which takes about 45 to 90 minutes. Here, Dr. Nichlos uses the modern operating rooms to operate gently using the modern technical possibilities of plastic and aesthetic medicine. Depending on need, the specialist performs the procedure under local anesthesia or twilight sedation.
After he has marked the areas to be treated and the anesthesia has taken effect, he injects a special solution into the tissue. It has a local pain-relieving effect and softens the area to make the subsequent suction of the sweat glands easier.
After a short exposure time, Dr. Nichlos inserts very fine cannulas through small skin incisions to suction out the sweat glands located directly under the skin in the subcutaneous fatty tissue. As a rule, suction curettage can remove 95 to 98 percent of the sweat glands responsible for the excessive sweating, so that sweating can subside and limits you less in everyday life.
After the sweat gland removal, Dr. Nichlos closes the small skin incisions with self-dissolving sutures and applies a compression bandage for the first 24 hours after the operation.
Healing after sweat gland suction: What to keep in mind
You can go home immediately after sweat gland suction. It is best to have someone pick you up, as driving a vehicle with freshly treated underarms is difficult.
Swelling and mild bruising stay within limits in most patients and usually subside on their own within a few days. For mild pain, you can take over-the-counter pain relievers from the pharmacy.
At home, you should take it easy for the first 2 to 3 days and avoid any physical exertion if possible. After that, you can return to work. You are usually back to social life the very next day. You should, however, refrain from sports, tanning beds and swimming pool visits for at least 1, preferably 2 weeks.
In addition, you must wear a compression vest for the first 1 to 2 weeks after sweat gland suction to prevent swelling and a buildup of wound fluid. We will give you details on this after the procedure, as well as tips on massage. It can help loosen hardening in the underarm area.
To monitor the healing progress, Dr. Nichlos will arrange follow-up appointments with you, which take place at our practice in Frankfurt.
What are the risks of sweat gland suction?
The risks associated with sweat gland suction are low with experienced surgeons like Dr. Nichlos, but cannot be ruled out completely. As with any surgical procedure, postoperative bleeding, wound healing disorders or infections are possible. After the surgical removal of the sweat glands, mild redness, bruising or swelling is not uncommon and usually disappears on its own after some time. The same applies to numbness in the surgical area.

Frequently asked questions about sweat gland suction in Frankfurt
Is sweat gland suction painful?
Thanks to the local anesthesia, you feel nothing during the procedure. Afterward, a feeling of tightness and mild pain are possible. The cannulas are very fine, which makes the procedure particularly gentle.
How quickly does it take effect?
As a rule, the reduction in sweat production is noticeable immediately after the procedure. The full effect shows after a few days.
Are visible scars left?
The access points for the fine cannulas are minimal and are usually closed with self-dissolving sutures. The scars usually heal inconspicuously.
How long does the result of the treatment last?
Since the removed sweat glands do not grow back, the result usually lasts long term. Follow-up treatment is only rarely necessary.
Can the remaining glands increase sweating again?
In rare cases, remaining glands may work more to compensate. However, the effect is usually small and does not bring sweating back to its original level.
Does health insurance cover the cost of the procedure?
Since Dr. med. Nichlos does not have statutory health insurance accreditation, billing through statutory health insurance is not possible. The cost of sweat gland suction must therefore be borne privately in every case. Even with a medical indication, statutory health insurance does not provide reimbursement.
Does sweat gland suction also help with sweating on the hands and feet?
Sweat gland suction is performed for heavy sweating in the underarms. For the hands and feet, this page describes treatment with Botox as an alternative: it can also be used in other affected areas, and the effect usually lasts 3 to 6 months. Dr. Nichlos will discuss which method is an option for you in the consultation.
