Breast augmentation Frankfurt

Natural breast augmentation with implants

For many women, a beautifully shaped bust is an important part of their femininity and plays a major role in their self-confidence. If they feel it is too small, an asymmetrical shape bothers them, or it simply does not match their ideal, their satisfaction and self-esteem suffer. Women often no longer feel comfortable in their own skin and hide their décolleté.

With breast augmentation, modern plastic and aesthetic surgery offers the possibility of adapting the natural bust in size and shape to individual wishes. The goal is for you to feel comfortable and feminine in your body and to wear your favorite clothes to your heart’s content.

A breast augmentation, whether with implants or autologous fat, requires great expertise so that the bust blends harmoniously into your silhouette. You are in good hands with Dr. med. Nichlos. The experienced specialist in plastic surgery performs the procedure regularly and is happy to advise you without obligation on the options available in your case for an augmentation.

What is a breast augmentation?

In breast surgery, breast augmentation, also called mammary augmentation, is primarily used to build up breast volume in breasts perceived as too small or sagging and to balance breasts of different sizes. It can also be used to reshape the breast in a targeted way and give the patient a new body feeling through a more feminine silhouette. We offer guidance on the right size in our guide Which cup size is right for me?

Specialist for breast augmentation in Frankfurt

Dr. med. Emmanouil Nichlos has specialized in breast augmentation for more than 17 years and has extensive experience in aesthetic breast surgery.

With a keen sense of proportion and natural shapes, Dr. Nichlos achieves aesthetic and harmonious results. Dr. Nichlos personally cares for freshly operated patients. In the first 24 hours after the operation, he is available to them for questions or problems.

What are the reasons for a breast augmentation?

The motivations for wanting a breast augmentation are very varied. By far the most common is that women simply find their breasts too small and therefore want a fuller, more feminine bust that provides a generous décolleté and makes aids such as push-up bras unnecessary.

Sometimes, however, a bust that has become slack or sagging through major weight loss or pregnancy is what prompts women to look more closely at breast augmentation and/or a breast lift.

Other reasons are asymmetries between the two breasts, which can often be balanced well with breast augmentation. After breast surgery (for example, tumor removal), a breast augmentation also offers the possibility of making up for the loss of volume and restoring the breast’s feminine shape.

Methods

What methods are there for breast augmentation in Frankfurt?

For breast augmentation, we use two methods proven in plastic surgery: implants and autologous fat. Both procedures have their specific advantages. Which method is best suited to you depends, among other things, on the degree of enlargement you want.

Breast augmentation with implants

With this method, we insert silicone implants (usually under the chest muscle). Depending on the breast shape or size you want, we can use different implants. Besides the round implant, more and more patients are asking for teardrop-shaped implants, as these create a very natural and feminine bust shape.

Implants are particularly suitable for women with small breasts who want considerably more breast volume and/or a defined breast shape. Even with a very slim figure and little body fat, this method allows us to fulfill the wish for a larger bust.

Alternative without implants

Alternatively, volume can be increased using the body’s own fatty tissue. This is taken, for example, from the abdomen, hips or thighs, processed and then injected into the breast in a targeted way. The breast enlarged in this way can look natural and feel natural. A practical side effect: by removing fatty tissue elsewhere, that area can also benefit visually.

Lipofilling is suitable for women who want a small adjustment in volume and who decline the use of foreign material in the body.

You can find more information on our page about breast augmentation with fat transfer

Hybrid breast augmentation with implant and autologous fat

Implant or autologous fat? Sometimes the best solution is to combine both methods. In a hybrid breast augmentation, Dr. Nichlos first places the silicone implant. This provides the desired shape and volume. He then refines the details by injecting the fatty tissue previously taken from the donor site and processed, for example concealing visible implant edges. The advantages compared with an implant-only operation:

  • The breast looks more natural and also feels that way
  • The risk of rippling (wavy implant folds) is considerably lower
  • A smaller implant can be used, as the autologous fat contributes volume
  • As a side effect, the donor area (e.g., abdomen, hips, etc.) can be contoured

A prerequisite for the method is, of course, that enough donor fat is available. Dr. Nichlos checks this before the operation and clarifies with you what the best solution is for you.

Which incision does Dr. Nichlos use for breast augmentation?

Incision in the inframammary fold (inframammary) for breast augmentation

Incision in the inframammary fold (inframammary)

This is by far the most common approach in breast augmentation, and for good reasons. The incision, only about 3 to 4 cm long, runs in the natural fold beneath the breast. The scar generally heals inconspicuously. Through the surgical area, which Dr. Nichlos can see well, he can place the implant in the breast with millimeter precision. The milk ducts and nipple nerves remain untouched, so breastfeeding and sensation in the nipple are generally preserved.

Incision around the areola (periareolar) for breast augmentation

Incision around the areola (periareolar)

With this variant, Dr. Nichlos makes the incision in a semicircle along the lower edge of the areola. If healing proceeds without problems, the scar is generally inconspicuous later on. With this approach, however, it cannot be avoided that parts of the breast gland tissue are cut through. This increases the risk of sensory disturbances. The ability to breastfeed can also suffer. Dr. Nichlos therefore recommends this approach only in certain cases, for example when the areola is also to be reduced and family planning is complete.

Incision in the armpit (transaxillary) for breast augmentation

Incision in the armpit (transaxillary)

With the transaxillary incision, the breast itself remains completely free of scars. Here Dr. Nichlos makes the incision for access in the armpit fold. Through a small tunnel, he guides the implant to the right place. This variant is technically very demanding, but routine for Dr. Nichlos. However, when implants are replaced, access through the inframammary fold often has to be created. The transaxillary variant is suitable for women with very small breasts without a proper inframammary fold and for patients who, for aesthetic reasons, want a completely scar-free breast.

Implant placement

Which implant placement is right for me?

Implants can be positioned in the breast in different ways. Which placement suits you best depends on various factors such as your body type, the thickness of your breast tissue, whether and how intensively you exercise, and your aesthetic wishes.

Above the chest muscle (subglandular)

With this variant, Dr. Nichlos places the implant above the chest muscle and directly beneath the breast gland (subglandular). Since the muscle is not involved in this operation, healing is faster and there is less pain afterward. This implant position has another advantage: for athletic patients, it does not deform as easily during intensive chest training, as it lies above and not under the muscle and the muscle therefore exerts no pressure on the implant.

This placement is less suitable for slim women with little breast tissue of their own, as the implant edges may be visible and palpable. A hybrid breast augmentation can help here (see above under methods of breast augmentation).

Partially under the chest muscle (dual plane)

This is currently the most frequently used variant in breast surgery and is suitable for most patients. Dr. Nichlos places the upper part of the implant under the chest muscle, while the lower part lies under the breast tissue. This creates a very natural transition without visible implant edges and a very attractive roundness in the lower part of the breast.

Completely under the chest muscle (submuscular)

With this variant, the implant lies completely under the chest muscle (submuscular) and is fully enveloped by it. The implant and its edges are therefore less often visible and palpable. The risk of capsular contracture is also lowest with submuscular placement. However, the operation is more involved and the healing time correspondingly longer. Dr. Nichlos recommends this placement for patients who want a particularly natural result and do not do intensive strength training, as this can deform the implant.

What happens before the breast augmentation in Frankfurt?

Before your breast augmentation, you come to our practice in Frankfurt for a thorough examination, where you have a detailed consultation with Dr. Nichlos in a calm atmosphere.

The specialist in plastic and aesthetic surgery wants to learn why you have decided on a breast augmentation and what you hope to achieve from the operation. You discuss how extensive your breast augmentation should be and whether implants or autologous fat are the suitable method for you. On site, Dr. Nichlos can also show you before-and-after photos of breast augmentations already performed.

Depending on which option you choose, the course of the breast surgery changes. Dr. Nichlos naturally explains this to you, as well as possible risks and complications and what you should pay attention to before and after the breast surgery.

You will of course also want to know what costs a breast augmentation will involve. A flat price generally cannot be given. We will, however, prepare an offer tailored to you individually, in which you will find all costs clearly listed.

Procedure

Breast augmentation in Frankfurt: How does the operation with implants work?

Breast augmentation with silicone implants takes about 1 to 1.5 hours and is performed by us in our affiliated clinic exclusively under general anesthesia. Before anesthesia is induced, Dr. med. Nichlos first marks the zones for the operation. Once you are asleep, the actual operation begins. The specialist first makes the incisions to insert the implants into your breast. There are different approaches, meaning types of incision: most commonly, the incision is made in the inframammary fold. The advantage is that the incision is barely visible there later. For implants with a smaller volume, an incision at the edge of the nipple area is also possible. A third alternative is an incision in the armpit.

Regardless of the type of incision, a pocket is formed in the breast tissue during the breast surgery, into which the implants are inserted. These can be placed either under or over the chest muscle. Placement under the chest muscle has the advantage that the implants are less palpable later and allows a more natural shape. Placement over the muscle goes along with a shorter healing time, but is only possible with sufficient breast tissue.

Dr. Nichlos then inserts the implants under sterile conditions and carefully checks their position and symmetry. Finally, he closes the wound with self-dissolving sutures and applies a dressing and the compression bra.

Aftercare

Healing after breast augmentation: What to keep in mind

In the first days after your operation, mild swelling, bruising, a feeling of tightness and pain may occur in the area of your breast. These are completely normal in the first few days and generally subside within 1 to 3 weeks.

It is important that you wear a compression bra or support bra for 6 weeks after implants are inserted and, after fat transfer, wear the compression garment at the donor sites as continuously as possible. A drain is not necessary. The purpose of the compression garment is to support the healing process and help stabilize the surgical result. Please sleep on your back at first and do not put pressure on the breast.

Make sure not to lift or carry anything heavy for 4 to 6 weeks and avoid physical exertion (including sports). The next day, you are generally back to social activities. Be sure to attend the regular follow-up appointments at our practice in Frankfurt am Main.

Risks

What risks does a breast augmentation carry?

Every surgical procedure carries certain risks, and this also applies to breast augmentation, both with autologous fat and with classic silicone implants. Breast surgery, however, is a very frequently performed routine procedure, and because the surgical area is limited to skin, tissue and muscle, complications occur only in very rare cases. They can never be ruled out entirely, though.

General risks and complications during and after a breast augmentation include the usual anesthesia risk, postoperative bleeding, wound healing disorders and infections. Injuries to individual nerves or the chest muscle are also possible, but very rare. Expert performance by an experienced specialist such as Dr. med. Nichlos and compliance with strict hygiene guidelines can reduce these risks.

Long-term risks of breast surgery with implants include capsular contracture. In this condition, the tissue around the implant hardens. The reason is that the body perceives the implant as a foreign body and tries to isolate it with a solid capsule. In the worst case, the implants need to be replaced. Thanks to the use of modern implants, however, the risk of developing capsular contracture is low.

Other, very rare complications of breast augmentation can include:

  • The breast implant slipping downward (also called bottoming out). The breast volume then shifts downward and the nipple upward. This can occur, among other things, if the recovery period after the operation is not consistently observed.
  • Breast implant illness (BII for short), which can cause various symptoms in the body in women with silicone implants, ranging, depending on the individual case, from chronic fatigue (fatigue syndrome) to hormonal problems, skin irritation or heart palpitations.
  • Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL for short), a very rare form of non-Hodgkin lymphoma that has been linked to silicone and saline implants. BIA-ALCL affects only about 1 in 50,000 women.

With breast augmentation using autologous fat, hardening and lumps can also occur in rare cases. It is also possible that the transferred fatty tissue heals in unevenly, so that the volume is no longer uniform.

FAQ

Frequently asked questions about breast augmentation in Frankfurt

With implants, volume is created using form-stable silicone implants, which is ideal if a noticeable enlargement or a specific shape is desired. With lipofilling, fat is taken from the abdomen, hips or thighs, processed and injected into the breast. This method is mainly suited to a moderate enlargement and offers a natural feel, with no foreign body at all.

Under the German Health Services Advertising Act (HWG), publicly showing before-and-after photos for aesthetic plastic procedures such as breast augmentation is not permitted.

During a personal consultation, you have the opportunity to view anonymized photo documentation from previous treatments. This gives you a realistic impression of possible results and a better idea of the procedure. You are also welcome to bring your own example photos so we can discuss your ideas and tailor the planning to your wishes.

Modern implants are durable and, if there are no complications, do not need to be replaced routinely. Breast augmentation with autologous fat also generally gives a permanent result, with about 60 to 80% of the injected fat remaining in the long term. How long the result stays visible also depends on age, skin elasticity and lifestyle.

Under general anesthesia, you feel nothing during the procedure. In the days afterward, you may experience a feeling of tightness or mild tenderness, especially if the implant is placed under the chest muscle. These symptoms can be managed well with pain medication. Most patients are back to social activities the next day.

Yes, for breast augmentation you can choose between the classic round implants and the newer teardrop-shaped breast implants. The latter are meant to give the breast a particularly natural look thanks to their anatomically correct shape. When deciding for or against one of the implants, however, your body proportions should be taken into account.

No, with modern silicone gel implants, like those we use in our practice, replacement is generally no longer necessary. It used to be said that breast implants last about 10 to 15 years and should then be changed. This generally no longer applies to today’s breast implants. As long as no problems occur, they can last many years. In individual cases, replacement may still become necessary. It is nevertheless advisable to have them checked regularly.

Your breasts feel hardly any different after augmentation than before, especially if we use autologous fat. Since no foreign body in the form of an implant is placed in the breast in this case, it continues to feel very soft and natural. If implants are to be used, Dr. Nichlos uses special surgical techniques to position them so that they are neither visible nor palpable, if possible. Only in naturally very slim women with implants that are large in relation to the breast gland tissue can it happen, in rare cases, that the implants can be felt.

Yes, in most cases breastfeeding is still possible after a breast augmentation. Implants are placed behind the breast gland or the chest muscle and generally do not affect the ability to breastfeed. With augmentation using autologous fat as well, the function of the breast is generally preserved.

That depends on your starting shape and your body proportions. With autologous fat, it is about half a cup size to one cup size. With implants, you choose the size together with Dr. Nichlos according to your wishes and to what your breast can carry well. To learn how cup sizes differ, read our article Which cup size is right for me?
