Removing an enlarged male breast: gynecomastia treatment with Dr. Nichlos

A defined upper body with a flat chest, or one slightly accentuated by the muscles, is generally considered the beauty ideal for men. A clearly protruding or even feminine-looking chest, as occurs with so-called gynecomastia, does not fit this picture. It can result from fat accumulation (pseudogynecomastia) or from enlargement of the mammary gland (true gynecomastia).

For affected men, an enlarged breast is often a major burden, both for aesthetic and for psychological reasons. They cannot identify with the female breast shape when looking in the mirror, deliberately avoid situations in which they have to show their bare chest (for example swimming pool, sauna), and avoid form-fitting clothing.

Gynecomastia surgery can help men who have already tried in vain to get rid of their female-looking breast by changing their diet and exercising. Depending on severity and cause, Dr. med. Nichlos removes excess fat deposits by liposuction or removes the enlarged mammary gland as part of the breast correction. The specialist in plastic and aesthetic surgery has many years of experience treating gynecomastia in Frankfurt and is glad to accompany you on the way to a more defined and more masculine chest. Schedule a consultation now!

What is gynecomastia?

Gynecomastia (colloquially “male breasts”) refers to a marked enlargement of the glandular breast tissue with a protruding nipple in men. This true gynecomastia must be distinguished from so-called pseudogynecomastia (lipomastia), in which the male breast results from significant overweight and consists mainly of fatty tissue.

What causes gynecomastia?

While pseudogynecomastia is the result of significant overweight, the causes of true gynecomastia have not yet been conclusively clarified. It is known that various factors favor “male breasts“. These include an imbalance of certain hormones, which can be traced back to age-related changes in the hormone balance.

Gynecomastia in boys going through puberty (pubertal gynecomastia) is relatively common, for example. It affects almost half of all male adolescents, but is usually only temporary and typically resolves on its own by the end of puberty at the latest, once the hormones are back in balance.

Enlargement of the male breast can also occur as a side effect of certain medications. In theory, gynecomastia can also point to an underlying condition (for example thyroid, kidney, or testicular disease, or breast cancer), but this is very rarely the case. Nevertheless, careful diagnostics are of great importance for targeted treatment.

In practice, however, it often happens that the causes of gynecomastia cannot be clearly identified despite detailed diagnostics. This is called idiopathic gynecomastia.

What treatment methods are there for gynecomastia?

With pseudogynecomastia, weight loss usually helps to make the “male breasts” disappear or at least shrink them significantly. If the breast is still too large at close to normal weight, liposuction can remove the excess fat cells. It does not replace prior weight loss, however.

True gynecomastia calls for breast surgery, as the excess glandular breast tissue has to be removed surgically. Dr. Nichlos performs the breast correction through an incision in the area of the nipple. During the operation, the nipple can often also be reduced to restore the proportions between breast and nipple. You can read more on the page Nipple correction.

In some patients, the operation is combined with liposuction, which also removes the surrounding fatty tissue. In very pronounced gynecomastia, additional incisions and subsequent skin tightening may be required.

What happens before gynecomastia surgery?

First, it needs to be clarified whether it is true gynecomastia or pseudogynecomastia and whether its causes can be identified. This requires various examinations in advance and a detailed medical history. Dr. Nichlos then informs you in a personal consultation about suitable treatment options and that the procedure is performed under general anesthesia.

If the examination shows that you have true gynecomastia and surgical removal of the excess glandular breast tissue makes sense, you receive comprehensive information about the course of the operation. During the consultation, Dr. Nichlos also informs you about possible risks and complications of gynecomastia surgery and explains which expectations of the operation are realistic.

You also receive information about what you need to observe before and after the procedure, such as avoiding alcohol and nicotine if possible and not taking any blood-thinning medication for at least 14 days beforehand.

Note: if you suffer from allergies or other chronic conditions and/or take medication regularly, please be sure to tell us early.

Procedure

Gynecomastia surgery in Frankfurt: how does the procedure work?

The operation is performed as an outpatient procedure, with an overnight stay if needed, in the modern operating rooms of our affiliated private clinic in Frankfurt under general anesthesia.

The incision placement can also vary in breast correction and can run at the nipples, along the breast fold, or in the armpits. Through the incision(s), Dr. Nichlos carefully removes the enlarged glandular breast tissue on each side. If needed, excess fat is also suctioned out. If too much excess skin remains afterward that can no longer retract on its own, it must also be removed and the remaining skin sutured. A drain makes it easier for wound fluid to drain off.

A compression dressing is then applied, which supports the new breast shape and its healing. The procedure to correct gynecomastia in Frankfurt usually takes about 1.5 to 2 hours.

Please follow our aftercare instructions and be sure to attend the follow-up appointments, so that Dr. Nichlos can monitor the healing process. In rare cases, a correction or follow-up treatment may be required to achieve the desired shape.

Healing after gynecomastia surgery: what to keep in mind

The drains are usually removed the day after the operation or the day after that. You then wear a compression vest for 4 to 6 weeks so that surgery-related swelling subsides more easily and the new breast shape can stabilize.

Avoid physical exertion during this time. You should only do sports after 4 to 6 weeks at the earliest, and in consultation with Dr. med. Nichlos. You are usually socially presentable again the next day. In any case, you should start with light exercises that put little strain on the upper body and cause no pain. Protect the scars from direct sunlight in the following months to avoid hyperpigmentation. We are happy to give you tips on proper scar care as part of aftercare.

Risks

What risks does gynecomastia surgery involve?

Complications are very rare when gynecomastia surgery is performed professionally. Like any surgical procedure, however, gynecomastia surgery involves certain general risks, including bleeding, wound healing disorders, and infections. These can be reduced by an experienced surgeon like Dr. med. Nichlos and strict hygiene standards.

Swelling, bruising, a feeling of pressure, or pain in the surgical area can occur as normal reactions to the procedure. Cooling and common pain relievers can treat these until they subside on their own within a few days.

Cost

To financing

What is included in the price?

The total cost of gynecomastia surgery consists of several components. These usually include:

  • detailed consultation (€100, fully credited if you go ahead with treatment)
  • individual treatment planning
  • surgeon’s fee for the operation
  • medical care during the treatment
  • compression vest for the healing phase
  • aftercare and follow-up appointments

In addition, the operation involves anesthesia costs and clinic fees, as the procedure is performed in a specialized clinic. If an overnight stay in the clinic is planned, depending on your individual recovery or at your personal request, its cost is added.

Many patients want to know before treatment what costs are involved in gynecomastia surgery. Because every finding is different, the exact price can vary depending on the severity of the gynecomastia and the scope of the procedure.

At our practice in Frankfurt, the cost of surgical treatment of gynecomastia usually starts at:

  • Gynecomastia surgery: from €4,500

The exact cost depends, among other things, on whether only excess fatty tissue needs to be removed by liposuction or whether enlarged glandular tissue is also removed. After a personal examination, Dr. med. Nichlos prepares an individual treatment and cost plan that lists all services transparently.

FAQ

Frequently asked questions about gynecomastia in Frankfurt

The incisions are placed as inconspicuously as possible, usually at the edge of the areola and, depending on the findings, also in the breast fold or in the armpit. The scars usually fade over time and are later barely visible.

The removed glandular tissue does not usually grow back, so a recurrence of true gynecomastia is generally not to be expected. However, renewed enlargement of the breast can result from unrecognized hormonal causes, certain medications or anabolic steroids, or significant weight gain. If there is an unrecognized hormonal cause, a different treatment is recommended. A stable weight and a healthy lifestyle help secure the result in the long term.

That depends on whether it is true gynecomastia or pseudogynecomastia. Only in the latter can a reduction through weight loss and exercise be possible. Endurance training and strength training are especially suitable for this. They help break down fatty tissue in the chest and strengthen the chest muscles. Enlarged glands or glandular tissue cannot be reduced through exercise alone, however, so it does not lead to the desired changes in true gynecomastia. Only gynecomastia surgery helps here.

Because gynecomastia surgery at Dr. med. Nichlos’ practice is a private medical service, billing through statutory health insurance is not possible. The costs must therefore be paid by you in any case. For privately insured patients, partial or full reimbursement may be possible in individual cases. Please check with your insurer in advance.

In true gynecomastia, the glandular breast tissue enlarges, often with a protruding nipple. Pseudogynecomastia (lipomastia), on the other hand, consists mainly of fatty tissue and occurs with significant overweight. Which form you have is clarified by Dr. Nichlos through an examination and a detailed medical history. This is important because, in very rare cases, a medical condition can be behind gynecomastia.

In boys going through puberty, gynecomastia is relatively common and usually resolves on its own by the end of puberty at the latest. With pseudogynecomastia, weight loss usually helps. True gynecomastia in adults, on the other hand, can only be treated surgically. If you are unsure which form you have, talk to Dr. Nichlos.
